Change of Module Form
You must fill in a "Change of Module" form if you want to change from one
optional module to another. It is an orange form and is available outside
the Faculty Office.
Fill in the form and either hand it in to the Faculty Office or the
Faculty Office PostBox (if there is no-one there).
Please make sure you fill out the form correctly, with:
- your name
- your id number
- program (write CA)
- year (write 1)
- the module(s) to be deleted (i.e. the module you are changing from)
Write each module code, the name of the module, full time, period 1 or
2 (for first or second semester). Most of you will put N (Normal) for
Attendence Type.
- the module(s) to be added (i.e. the module you are changing to)
The same information is required here also.
- your signature
- the date
The Faculty Office will ensure that it is signed by the Chairperson.
The Faculty Office will take care of it from there.

Attending lecturers/tutorials/labs for your new module
You should attend your new module once you have submitted the form. It is
your responsibility to catch up on any course material that you may have
missed. You will be assigned a new practical/lab group (where appropriate)
in due course - in the meantime, please attend a convenient practical/lab
group and inform the tutor that you have transfered into the module.
